Eligibility Criteria

-A physician's order (letter or prescription) is required, must include the following:
medical reason for liquid supplementation, product type, number of cans to be consumed per day, current weight.
-Must be renewed annually.
-Must also be a Bloomfield Township resident to receive this service.
